Commissioner of Oaths

Members of the public may require the commissioning of documents whereby one is required to take an oath or an affirmation or make a declaration in front of a Commissioner. A Commissioner is available by appointment Monday to Friday from 8:30 a.m. to 4:30 p.m.

To book an appointment, please call 519-369-2200 ext. 243. The fee for commissioning documents is $25.

Important: Please complete your document before visiting our office but do not sign the affidavit section of the document.

At our office you must:

  • present your completed document;
  • present one valid piece of original government-issued photo identification (i.e. driver's licence, passport, new health card*, or a BYID age-of-majority card) showing your name and signature;
  • sign the affidavit section of the document in the presence of the Commissioner.

All parties required to sign the document must be present at the commissioning appointment. All documents must be presented in English.

Please note that we do not have a notary public on staff and do not notarize documents.

*The Municipality of West Grey does not accept health cards as a form of photo identification for any other service.

There is no guarantee that a document can be commissioned until it is seen by the Commissioner.

The types of documents that can be commissioned by staff include, but are not limited to the list below:

  • delayed birth registration forms
  • application to amend birth registration
  • election to change of name of child under the age of 12
  • statutory declaration by an applicant to correct an error in registration
  • consent letter for traveling with a child commonly referred to as a travel document
  • legal name change application
  • insurance claims
  • Statutory Declaration for Ontario Student Assistance Program (OSAP) – Statement of Common Law Status/Supporting Children
  • vehicle ownership transfers
  • invitation letters for a visitor’s visa
  • Statutory Declaration In Lieu of Guarantor – for Canadian Passport
  • affidavit waiving vaccinations
  • Statutory Declaration of Lost Identification
  • West Grey planning applications
  • pension purpose attestations of recipient being alive and confirming their residency

Under the  Commissioners for taking Affidavits Act, R.S.O. 1990, c. C. 17, staff are not permitted to commission the documents listed below:

  • age of majority/Ontario photo ID applications
  • any documents requiring the signature of a guarantor (document will note this requirement)
  • court and/or legal documents (related to civil issues)
  • custody documents
  • declaration of claimant and indemnity
  • declaration for replacement of shares
  • divorce, separation, marriage, or cohabitation documents
  • documents noting the signature of a Notary Public is required
  • estate settlement documents
  • incomplete documents
  • international educational institution or association documents, including photographs
  • Ministry of Children, Community and Social Services Family Responsibility Office forms
  • powers of attorney
  • real estate documents, including rental lease agreements
  • transfers of money or estates of a deceased person
  • wills, living wills, codicil to a will

Commissioner of Oaths have the right to refuse commissioning services on any document.
