Members of the public may require the commissioning of documents whereby one is required to take an oath or an affirmation or make a declaration in front of a commissioner. A commissioner is available by appointment Monday to Friday 8:30 a.m. to 4:30 p.m. To book an appointment, please contact us at 519-369-2200 ext. 243.
Important: Complete your document before visiting our office. Do not sign the affidavit section of the document.
At our office you must:
- present your completed document;
- present one valid piece of original government-issued photo identification (i.e. a valid driver's licence, a valid passport, new health card or a BYID age-of-majority card) showing your name and signature;
- sign the affidavit section of the document in the presence of the Commissioner.
All parties required to sign the document must be present at the commissioning appointment. All documents must be presented in English.
Please note that we do not have a notary public on staff and do not notarize documents.

Under the Commissioners for taking Affidavits Act, R.S.O. 1990, c. C. 17, staff are not permitted to commission the documents listed below:
